The hormone lever

Your blood test first, then working through whatever is actually suppressing you: sleep debt, body fat, a real deficiency, alcohol, untreated sleep apnoea, or something on your medication list.

What we work on: sleep, body fat, training, alcohol, stress and any genuine deficiency your results point to. Your retest shows what changed, if anything.

The function lever

Structured pelvic floor training, contraction and relaxation both. Unguided kegels off the internet often make things worse, because an over-tight pelvic floor causes problems of its own.

What we work on: pelvic floor strength and control, which research links to erection firmness and ejaculatory control.

Pelvic floor training does not raise your testosterone. We include it because many men asking about testosterone also want help with erections, and that is a separate job.

Before you book

  • Go between 7 and 10am, fasting.
  • Stop biotin and hair, skin and nail supplements 72 hours before.
  • Don't test while unwell or within 48 hours of heavy training.
  • Expect to repeat it on a second morning before drawing conclusions.
Total testosterone
The headline number. Ask for LC-MS/MS if your lab offers it.
SHBG and albumin
Needed to calculate free testosterone. Without them, total explains little.
LH and FSH
Show whether the signal starts in the brain or the testes. Often left off.
Oestradiol
Testosterone converts to it in fat tissue. Useful context.
Prolactin
High prolactin with low LH needs a doctor, not a programme.
25-OH vitamin D
Worth correcting only if it's genuinely low.
Ferritin
Iron overload can lower testosterone. Never take iron blind.
HbA1c and fasting glucose
Insulin resistance suppresses testosterone. Often the most useful number.
TSH and free T4
Thyroid problems mimic low testosterone almost exactly.
Full blood count, liver, kidney
Baseline safety, including haematocrit.
PSA
Talk to your GP about whether it's right for you, especially from 50, or earlier with a family history.
